Seymour Elementary Parents
The Illinois 5 Essentials Survey is currently collecting feedback from parents, teachers and students. The survey can be completed on a computer or a mobile device, including your phone, in about 10-15 minutes. In addition to having a direct impact on the learning experiences and outcomes of your students at school, finishing the survey will allow you to be entered in a drawing for a chance to win one of two $25 Technology Script Cards.
After completing the Seymour Elementary survey, parents and guardians should e-mail seymourelementary@gmail.com a screen shot of the final page of the survey or forward the “Thank you” e-mail you receive from UChicago Impact Surveys. Please include your name and your student(s) name(s).
Please make sure you search for Seymour, not Payson when typing the name of the school.
